No one responsible, someone is always responsible

Systemic failures…a life lost…no one accountable (KN Mar 29). No justice for the family.
How could no one be held accountable or responsible when failures were established and recognized? Failures were man made as it is people who have to implement and ensure systems work. So, someone must be responsible and accountable.
Too often many failures are laid bare during investigations with promises to rectify and improve until the next time. And the cycle starts all over again. The buck stops with the hierarchy of the medical care staff of the hospital. They should be held accountable and either resign or be relieved of their positions. With all the monies pumped each year into the health care system for failures to be happening tells of poor management and accountability. This is a slap in the face of those tasked to ensure gaps are taken care of and do not occur. After all lives are endangered.
To say no one is accountable is not accepting responsibility failures the hospital was supposed to recognise and put corrective measures and systems in place in order to prevent this tragedy from happening. It didn’t and the poor consolation is ‘systemic gaps’ contributed to the unfortunate outcome.
Unacceptable, pure nonsense.
